Middle School E-week Health Forms and Preparation
As we look ahead to E-weeks (Tuesday, May 26 - Friday, June 5), we’re excited for this meaningful time when students step beyond the classroom to learn through experience-building independence, strengthening community, and embracing new challenges together.
To help ensure every student can fully participate in a safe and supported way, we ask that all required health information is up to date by Friday, May 15.
Medication and Forms
If your child takes medication during the school day or may need it during E-weeks, please update their information in August Schools.

- All medications require a completed and signed form from a healthcare provider
- A separate form is required for each medication
- Blank forms are available on the Health Information Resource Tile in the Community Portal

These preparations allow our team to care for students thoughtfully and respond effectively, so they can fully engage in the hands-on, place-based learning that defines E-weeks.
